All rates shown reflect a monthly premium but Rochester Chamber will bill quarterly.  A detailed plan summary for each plan can be accessed by clicking on the plan name.

Please keep in mind that the Affordable Care Act mandates that pediatric dental is required for subscribers who have dependents under the age of 19. If you have a stand-alone dental plan that fulfills the mandate, you can purchase your medical plan without the pediatric rider.  MVP will automatically add the rider to any subscriber with dependents under age 19.  To opt out, it must be indicated on your enrollment form.  Pediatric dental riders are available for MVP members through Delta Dental or MVP Dental for Kids.

All plans include dependent coverage to 26.  If you would like to purchase the dependent to age 30 rider, please call for pricing.

To enroll in an MVP health plan, you must download the appropriate new group enrollment forms packet listed below. Once you have downloaded the forms packet, you will also need to download a rate sheet for the particular plan you have selected. The rate sheet link is attached to the plan summary found at the plan comparison and premium links above.
